Which ones should have happened?

The patient arrived by ambulance during an active trauma activation. The physician was on shift and the patient was in the emergency department census at the moment of access. Nothing here needs a privacy officer’s time.

A shared last name and a shared household address point to a family relationship. No encounter connected the clerk’s department to this record when it was opened. This is the case of a family-member access, and it warrants a conversation.

The record had no open encounter, the patient was treated on a different unit, and the access falls inside a notice period already recorded in the identity system. All three facts are available at the moment of access, not weeks later in a retrospective audit.

Every one of the fourteen records sits on the worklist assigned to that technologist for that shift, each with an imaging order attached. High volume across departments in a short window is the normal working pattern for an imaging role. Flagging it produces noise, not findings.
